NOTICE OF PRIVACY POLICY AND INSURANCE INFORMATION PRACTICES FOR SNOWDEN & ASSOCIATES, INC.

 

We at Snowden & Associates, Inc. value our relationship with you and we realize that in order to properly manage your insurance needs, we must handle private information for you and/or your employees. At times we must share this information with the insuring company and the third party administrator, but we will not share with any party this is not vital to the processing of the insurance coverage. We also will not share employees’ private information with the employer. Please share this notice with your employees.  The following notice describes our policies and practices with regard to collecting and disclosing nonpublic personal information about our customers and former customers.

We maintain appropriate physical, electronic and procedural safeguards to maintain the confidentiality and security of your nonpublic personal information contained in our records.  We restrict access to nonpublic personal information about you contained in our records to our employees who need to know that information to provide products or services to you.

We will directly contact any affected party if there is a security breach impacting private health information. If more than 10 people are impacted we will notify local media and post a notice to our web site. If more than 500 people are impacted, we will notify Health & Human Services as well as well-know media outlets.

 

Changes to Our Notice of Privacy Policy and Insurance Information Practices

We reserve the right to change our privacy policies and insurance information practices.  If we make any changes to our policies or practices, we will provide you with a copy of a revised notice as required by applicable law.
